How to use "Tests Order Sheet" for Microsoft Excel

1. Please use Microsoft Excel.
2. Please input directly to the cells.
3. You need to input

　　＝　Statement of Test Fee and Others　＝
　　　- Number of Participants
　　　- Number of Extra CDs (Only when you ordered more than 2 CDs for each grade)
     [Member only]
      - Commission（If you are an Official member, imput 25. If you are a Partial Member, input 15.）
　　　- Annual Fee (Input 1 to the quantity cell)
　　＝　Others　＝
　　　- Test Date and Maturity Date
　　　- Mode of Remittance
        You can choose from Bank transfer (Tokyo-Mitsubishi UFJ Bank or Risona Bank), Postal Money Transfer, or Registered Postal Cash Envelope.

                      Risona Bank, Sannomiya branch, Touza 413457
　　　- Member ID(Member only), Name of the Classroom, Name, Address, Phone number

4. You don't have to culculate the total Payable.
5. If you save this file to your PC, you can re-use this sheet without input your name, address and other basic information.
